Do you live with a family member who has been diagnosed with prediabetes or type 2 diabetes? Do you have questions that have not been answered no matter where you look for information? Are you overwhelmed or frustrated with the care that you want to provide for your loved one but do not understand?

Thank your for visiting my site. It is my goal to provide a tool that you, the family member of a patient with diabetes, can use as a source of information for caring for your loved one.
I appreciate you taking the time to not only complete my project but more importantly, being an active participant in the chronic care of your loved one.
Please follow the steps in the top right corner, starting with a quick survey. The education should take approximately 30 minutes to complete and follow up surveys will be sent to you 1 month and 3 months from day you complete the pre-survey. Emails provided for the survey will not be kept for use beyond sending survey reminders.
